Abstract

INTRODUCCIÓN Y OBJETIVOS: El trastorno bipolar (TB) tiene una prevalencia del 2,4%, siendo más diagnosticado el tipo I (0,6%) que el tipo II (0,4%), estimando un 1,4% en grupos subumbrales. El diagnóstico de TB tipo II es más díficil e incluso se demora más de 10 años puesto que los episodios hipomaníacos pueden pasar desapercibidos como etapas de gran éxito y productividad, en las que el paciente no percibe malestar ni solicita ayuda. DESCRIPCIÓN DEL CASO: Varón de 52 años con antecedentes de hernia discal, espondilodiscitis secundaria y síndrome facetario lumbar. Cuenta con antecedentes conocidos de episodios depresivos en 2014 y 2022, ambos tras cirugía lumbar con limitación funcional significativa. Niega tóxicos. Presenta sintomatología compatible con episodio depresivo. Ante escasa mejoría, se cambia antidepresivo a Duloxetina y Trazodona. Posteriormente se observa clínica hipomaníaca que obliga a retirar Trazodona y pautar Olanzapina, que el paciente rechaza. EXPLORACIÓN Y PRUEBAS COMPLEMENTARIAS: Durante el último mes se objetiva un mayor nivel de actividad, expansividad e irritabilidad, aumento de conductas de riesgo y gastos, y nula necesidad de descanso. Dada la inestabilidad psicopatógica, se decide ingreso. DIAGNÓSTICO DIFERENCIAL Y TRATAMIENTO: Inicialmente sería planteable pensar en un viraje a episodio hipomaníaco causado por la toma de antidepresivos. Sin embargo, realizando una profunda anamnesis, el paciente refiere haber presentado etapas en las que se ha sentido particularmente bien, acelerado, con comportamientos desajustados y gastos excesivos, que podrían corresponderse con episodios hipomaníacos. Estos datos nos orientan a un TB tipo II. Se inicia tratamiento con Ácido Valproico y Aripiprazol, con tendencia a la estabilización anímica y normalización del sueño. DISCUSIÓN Y CONCLUSIONES: Muchos pacientes con síntomas depresivos en contexto de un trastorno bipolar son frecuentemente mal diagnosticados de episodio depresivo mayor dada la dificultad de diagnosticar retrospectivamente. Resulta fundamental preguntar de manera rutinaria por posibles síntomas de manía/hipomanía previos en cada paciente con un episodio depresivo mayor para detectar y tratar de manera adecuada, así como trabajar la conciencia de enfermedad en personas ya diagnosticadas y conocer los signos de alarma para evitar recaídas.
