Abstract

Resumen Introducción: no existe un registro nacional sobre dislipidemia y metas de colesterol de lipoproteínas de baja densidad (LDL-c) por grupos de riesgo de enfermedad cardiovascular ateroesclerosa (ECVA) enfocado en derechohabientes del Instituto Mexicano del Seguro Social (IMSS). Objetivo: determinar la frecuencia de dislipidemia, metas de LDL-c y pacientes en tratamiento de grupos de alto y muy alto riesgo de ECVA. Material y métodos: estudio multicéntrico, transversal, descriptivo. Este artículo deriva del Registro Mexicano de Dislipidemias en pacientes de alto riesgo y muy alto riesgo de enfermedad cardiovascular ateroesclerótica (Proyecto REMEXDIS-IMSS). Se incluyeron pacientes con criterios de alto riesgo y muy alto riesgo de ECVA. Resultados: de julio de 2022 a marzo de 2023 se incluyeron 6000 pacientes: 3289 pacientes del grupo de alto riesgo y 2771 del grupo de muy alto riesgo. La frecuencia de dislipidemia se observó en el 49% de la cohorte. El grupo de muy alto riesgo presentó mayor porcentaje de dislipidemia en 66.8%, infarto agudo de miocardio en 81.0% y angina de pecho en 21.9%. El uso de estatinas fue mayor en este grupo (93.3%) y la meta de LDL-c se logró en el 72.9% (p < 0.0001). Conclusiones: la frecuencia de dislipidemia se encontró en casi la mitad de la población estudiada. El grupo de muy alto riesgo tuvo mayor frecuencia de dislipidemia, mejor uso de estatinas para prevención secundaria y mayor número de pacientes en metas de LDL-c respecto al grupo de alto riesgo.

Background: There is no national registry on dyslipidemia and low-density lipoprotein cholesterol (LDL-c) goals by risk groups for atherosclerotic cardiovascular disease (ACVD) focused on beneficiaries of the Mexican Institute for Social Security (IMSS). Objective: To determine the frequency of dyslipidemia, LDL-c goals and patients in treatment from high and very high-risk groups of ACVD. Material and methods: Multicenter, cross-sectional, descriptive study. This article derives from the Mexican Registry of Dyslipidemias in patients at high-risk and very high-risk of atherosclerotic cardiovascular disease (REMEXDIS-IMSS Project). Patients with high-risk and very high-risk criteria for ACVD were included. Results: From July 2022 to March 2023, 6000 patients were included (3289 patients in the high-risk group and 2771 in the very high-risk group). The frequency of dyslipidemia was observed in 49% of the cohort. The very high-risk group presented a higher percentage of dyslipidemia in 66.8%, acute myocardial infarction in 81.0% and angina pectoris in 21.9%. The use of statins was higher in this group (93.3%) and the LDL-c goal was achieved in 72.9% (p < 0.0001). Conclusions: The frequency of dyslipidemia is found in almost half of the population.
