Abstract

Résumé Le présent document constitue un guide d’utilisation de deux matrices de maturité de l’intégration des soins. L’une est à l’usage d’un prestataire de soins (comme un Pôle Santé vaudois) : SCIROCCO-POLE. L’autre est à l’usage d’un canton suisse (ou un équivalent) : SCIROCCO-CANTON. Le guide traite principalement de SCIROCCO-POLE mais la mise en oeuvre de SCIROCCO-CANTON peut être naturellement déclinée sur la même base. Ces deux matrices s’appuient sur la structure de la matrice de maturité « Scaling Integrated Care in Context » (SCIROCCO). Développée dans le cadre d’un projet européen, elle vise à évaluer l’intégration des soins d’un système de santé national ou régional. L’intérêt des matrices de maturité est d’être un outil relativement simple et pragmatique de gestion et de pilotage. En effet, de par leur nature, elles permettent de réaliser un diagnostic qui, en creux, identifie les éléments clés d’une stratégie d’amélioration. Cette dernière peut ensuite être élaborée en utilisant une feuille de route, détaillée avec des plans d’actions précis. Les deux matrices décrites dans le guide, à savoir SCIROCCO-POLE et SCIROCCO-CANTON, ont été développées dans le cadre d’un projet de recherche appliquée qui a rassemblé deux équipes de recherche, spécialistes du management et de la gestion dans le secteur de la santé, et les trois Pôles Santé vaudois. Les objectifs de ce document sont triples : Présenter la démarche méthodologique qui a conduit à élaborer les deux matrices de maturité ; Mettre à disposition des lecteurs ces deux matrices ; Transmettre l’expérience acquise lors de la mise en oeuvre de la phase d’évaluation. Concernant le 3ème objectif, le guide explicite de manière détaillée les ressources nécessaires ainsi que les différentes phases d’un tel processus. Pour chaque phase, il détaille les objectifs, les actions à réaliser, des recommandations sur lesquelles s’appuyer, les responsabilités des acteurs et les livrables attendus. AbstractThis document serves as a guide for utilizing two maturity matrices designed for integrated care assessment. One matrix, SCIROCCO-POLE, targets care providers such as the Pôles Santé Vaudois, while the other, SCIROCCO-CANTON, is tailored for use by Swiss cantons or equivalents. While the guide primarily focuses on SCIROCCO-POLE, the adaptation of SCIROCCO-CANTON follows a similar approach. Both matrices are structured based on the «Scaling Integrated Care in Context» (SCIROCCO) maturity matrix, developed as part of a European project to evaluate care integration within national or regional healthcare systems. The benefit of such maturity matrices lies in their simplicity and practicality as management and steering tools. They facilitate diagnosis, pinpointing key areas for improvement, and subsequently enable the development of detailed improvement strategies with precise action plans. The development of SCIROCCO-POLE and SCIROCCO-CANTON stemmed from an applied research project involving two specialized healthcare management research teams and three Vaud care providers (Pôles Santé Vaudois) This document aims to achieve three objectives: Presenting the methodological approach leading to the development of the two maturity matrices. Providing access to these matrices for readers. Sharing the experience gained during the assessment phase implementation. In addressing the third objective, the guide elaborates on the required resources and various phases of the assessment process. Each phase is outlined with its objectives, actions, recommended bases, responsibilities of stakeholders, and anticipated deliverables.
