Abstract

La enfermedad celíaca (EC) es una patología autoinmune desencadenada y perpetuada por la ingesta de gluten en individuos genéticamente susceptibles, en donde se produce daño en la mucosa intestinal principalmente de intestino delgado y condiciona múltiples y variables manifestaciones clínicas. Los criterios diagnósticos incluyen la historia clínica, determinación de anticuerpos específicos en suero y biopsia intestinal. La determinación de laboratorio ha tomado cada vez más auge debido a la elevada sensibilidad y especificidad de ciertos marcadores como la anti transglutaminasa tisular IgA (tTG IgA) y los anticuerpos anti endomisio (EMA) los cuales, junto con las manifestaciones clínicas , permitirían establecer con certeza el diagnóstico si sus concentraciones séricas se encuentran por encima de 10 veces el valor máximo de la normalidad en el caso de la tTG IgA, con lo cual se evitaría la realización de biopsia intestinal en los niños. Los marcadores genéticos específicos HLA DQ-2/ HLA DQ-8 se reservan para aquellos casos de diagnóstico dudoso, con serología negativa, para familiares en primer grado o en pacientes con dieta libre de gluten que no hayan sido previamente evaluados.
