Abstract

Resumo: o objetivo deste estudo foi avaliar a situação do aleitamento materno dos recém-nascidos de" muito baixo peso do Serviço de Neonatologia do CAISM-UNICAMP. Foram estudados 117 pacientes internados de abril de 1990 a maio de 1993, cujas mães participaram do programa de incentivo ao aleitamento materno. Neste programa, eram dadas informações sobre a importância do aleitamento natural, técnicas de ordenha, transporte, armazenamento e manutenção da produção do leite materno. Numa segunda fase, mãe e recém-nascido eram transferidos para o Alojamento Conjunto Tardio, ACT, para o início do aleitamento no seio. Quando a suplementação láctea era necessária, utilizava-se a técnica de relactação. A alta hospitalar ocorria" quando havia boa interação mãe-filho, com a criança mamando bem e em ganho de peso. O acompanhamento ambulatorial foi feito em 7 dias após a alta e, aos 3,6 e 12 meses de vida para avaliação da duração do aleitamento. Os resultados mostraram que 97,5% dos recém-nascidos estavam mamando ao seio na alta hospitalar. Na análise múltipla, somente o uso da técnica de relactação permaneceu associada, significativamente e de maneira negativa, ao aleitamento materno exclusivo na alta hospitalar. A taxa de continuação do aleitamento materno, calculada por tabela de vida, foi de 81,2% aos 3 meses, 43,3% aos 6 meses e 13,8% aos 12 meses de idade, com mediana de 5,37 meses. A duração do aleitamento foi maior nas crianças que não necessitaram de relactação e naquelas que receberam alta em aleitamento materno exclusivo. Na análise múltipla, apenas o tipo de aleitamento na alta hospitalar mostrou significação estatística

Abstract: The aim of this study is to evaluate the breastfeeding in very low weight neonates at the Neonatal Services of CAISM-UNICAMP. One hundred and seventeen patients, hospitalized between April 1990 and May 1993 whose mothers took part in the breastfeeding promoting program, were studied. In a first stage this program furnished' information on the importance of breastfeeding, milking techniques and storage, transport and maintenance of breast milk collected. During the second stage, mother and neonate were transferred to late rooming-in (Alojamento Conjunto Tardio - ACT) to breastfeed. The technique of relactation was used when supplementation was necessary. The discharge from the hospital was granted after observing a good mother-child relationship, the child was sucking well and weight was increasing. In order to evaluate the total period of breastfeeding, a follow up was carried out at 7 days after discharge, and when the child was 3, 6 and 12 months old. The results showed that 97.5% were breastfeeding when discharged from hospital. On discharge, logistic regression analysis showed that only the relactation technique was negatively related to exc1usive breastfeeding. The breastfeeding rate calculated according to the life table method was: 81.3%; 43.3% and 13.8% for the 3, 6 and 12 months old children, with a median of 5.37 months. Breastfeeding was longer for children who did not need relactation and those who, on discharge, were exc1usively breastfed. The only significant correlation shown by the multivariate survival data analysis was the time of lactation at the time of discharge from the hospital
