Сравнительный анализ удовлетворенности от лечения пациентов терапевтического профиля в клиниках Саратова и Массачусетса

Abstract

На сегодняшний день нет достаточного количества достоверной информации по проблеме удовлетворенности от лечения, несмотря на активное изучение этой проблемы. Весьма интересным является опыт американских коллег, в 1998 году проводивших научную программу Massachusetts Health Quality Partners (MHQP) Statewide Patient Survey Project посвященную этой проблеме. И так как подобных исследований на достаточно большой выборке пациентов в отношении российских больных не проводилось, то мы решили выяснить приоритеты отечественных пациентов, для чего в нашей клинике было проведено исследование по методике MHQP Statewide Patient Survey Project. Результаты, полученные в нашем исследовании, сравнивались с результатами указанной программы. В программе MHQP Statewide Patient Survey Project принимало участие 51 медицинское учреждение штата Массачусетс. По почте были разосланы анкеты случайно выбранным 27414 пациентам. Нами проводилось анкетирование пациентов, находящихся на стационарном лечении в Клинической больнице медицинского университета. Применяемая в обоих исследования анкета опросник Picker Institute questions. В нашем опросе согласились принять участие 150 пациентов. В программе MHQP Statewide Patient Survey Project всего получены ответы 12680 пациентов. По таким параметрам как уважение к пациенту, организация лечения, предоставление информации и обучение, физический комфорт удовлетворенность как наших, так и американских пациентов вполне сопоставима. Оценка эмоциональной поддержки несколько ниже у наших пациентов. Привлечение к процессу лечения семьи адекватным посчитали далеко не все пациенты, как в нашей клинике, так и в американских стационарах, причем у нас недовольных было достоверно больше. Наиболее уязвим процесс выписки из стационара, как для наших, так и для американских пациентов, хотя, вероятно, для отечественных больных этот момент является еще более критическим. В оценках стационарного лечения больными из разных клиник, разных стран очень много общего. Процесс выписки из стационара получил наиболее низкую оценку обследуемых пациентов и по результатам MHQP Statewide Patient Survey Project и по данным нашего собственного исследования. Представляется, что форма заключительной беседы и обстановка, в которой она проводится, нуждаются с одной стороны в более глубоком изучении, а с другой в значительной коррекции.

Despite numerous studies of patient satisfaction with treatment, they have not resulted in the enough authentic information about this problem. Massachusetts Health Quality Partners (MHQP) conducted the Statewide Patients survey to provide hospitals information about patientssatisfaction. Results of program were very interesting. We hadn't such studies in Russia and that is why we collected information in Clinical Hospital of Saratov Medical State University to reveal patients' mind about this problem in our country using the methods of MHQP Statewide Patients survey. Data from our study was compared with results of mentioned survey. MHQP Statewide Patient Survey Project obtained results from 51 hospitals across Massachusetts. 27414 questionnaires were-mailed. We conducted survey from in-patients of clinical hospital of medical state university. Hospital experiences were assessed with Picker institute question in both studies. 150 patients took part in our study. 12680 respondents constituted the sample for analysis in MHQP Statewide Patients survey. Patients' satisfaction with treatment with respect for patient preferences, coordination of care, information and education, physical comfort in our hospital was the same with Massachusetts results. Inclusion of family and friends was determinate by patients as insufficient as in our clinic, as in Massachusetts hospitals; moreover rate of satisfied patients in our hospital was lower than in Massachusetts. The most critical finding is low level of patients satisfied with continuity and transition after discharge from hospital in both countries, but especially in Russia. There are more common in patients' characteristic of in-hospital treatment in different countries. Evaluation of continuity and transition after discharge from hospital by patients was the lowest as in current study as in MHQP Statewide Patients survey. Further research is needed to measure specific aspect of doctors' last conversation with patient in hospital and some changes in this process should be done.
