Diferencias en la atención de las mujeres con alto riesgo cardiovascular respecto a los varones: estudio multidisciplinario

Abstract

Fundamento y objetivo: Existe un amplio espectro de pacientes con alto riesgo cardiovascular (ARCV) que pueden beneficiarse de medidas generales y farmacologicas. El objetivo del estudio fue evaluar las diferencias en la presentacion del ARCV entre mujeres y varones, y si existen diferencias de tratamiento en razon del sexo. Pacientes y metodo: Se estudiaron los datos de pacientes con ARCV en consultas de cardiologia, medicina interna, neurologia, endocrinologia y atencion primaria. Se consideraron pacientes con ARCV a aquellos con antecedentes de enfermedad coronaria, cerebrovascular, vasculopatia periferica y diabetes mellitus (DM) mas otro factor de riesgo cardiovascular. Las principales variables analizadas fueron: edad, sexo, glucemia, hemoglobina glucosilada, presion arterial, tabaquismo, dislipemia, microalbuminuria, asi como los farmacos utilizados en todos los pacientes. Se realizo un estudio multivariante ajustado por edad. Resultados: De un total de 5.027 pacientes evaluados, 1.307 presentaban ARCV (56,1% varones, 43,9% mujeres) con una edad media de 67,3 anos (varones: 66,1; mujeres: 68,8 anos). Entre los pacientes con enfermedad coronaria, en las mujeres se prescribio menos antiagregacion respecto al varon (69,4 frente al 80,4%; odds ratio [OR] = 1,592) y menos hipolipemiantes, a pesar de existir una mayor prevalencia de hipercolesterolemia asociada (52,8 frente al 39,1%). Entre los pacientes con DM y otro factor de riesgo vascular, las mujeres recibieron menos antiagregacion (42,9 frente al 36,6%; OR = 1,486) e hipolipemiantes (53,5 frente al 41,4%), y mas diureticos (41,4 frente al 26,5%; OR = 0,588). Conclusiones: Existe un diferente perfil de ARCV en la mujer respecto al varon, con mas DM y menos tabaquismo. Se observa una tendencia a infratratar a la mujer con ARCV respecto a las pautas establecidas en el varon.
