Abstract

Resume Introduction Le syndrome de Wells, ou cellulite a eosinophiles, est une dermatose inflammatoire rare de pathogenie inconnue, caracterisee par un aspect clinique de dermo-hypodermite prurigineuse le plus souvent localisee aux extremites avec, a l’histologie, un infiltrat dermique constitue essentiellement d’eosinophiles associe a des images « en flammeches ». Observations Nous rapportons trois observations de syndrome de Wells idiopathique presentant un tableau clinique trompeur mimant une cellulite infectieuse de la face. La prise en charge therapeutique, reposant sur la prednisone et/ou l’association d’antihistaminiques levocetirizine/hydroxyzine, a permis dans tous les cas une amelioration rapide des lesions et le maintien d’une remission clinique prolongee. Discussion Ces trois observations soulignent l’importance d’evoquer le diagnostic de syndrome de Wells devant un aspect de cellulite aigue du visage ne repondant pas a un traitement antibiotique adapte. Nos observations suggerent egalement que l’association d’antihistaminiques, plus particulierement l’association levocetirizine/hydroxyzine, est une alternative therapeutique interessante en premiere intention ou en relais d’une corticotherapie generale.
