Tratamiento mediante dilatación endoscópica de las estenosis esofágicas cáusticas

Abstract

Objetivo Evaluar los resultados de la dilatacion endoscopica en las estenosis esofagicas causticas y analizar los factores asociados a una buena respuesta. Pacientes y metodo Estudio restrospectivo sobre 33 enfermos sometidos a dilatacion con bujias de Savary entre 1989 y 2001. Se analizaron la respuesta a la dilatacion inicial y la evolucion durante el seguimiento. Resultados La edad media fue de 50 anos (8-83), con un 58% de mujeres. La ingesta fue accidental en 29 (88%). En todos los casos el caustico ingerido fue alcali. La dilatacion se inicio en fase aguda en 12 pacientes (36%) y 13 presentaban fenomenos inflamatorios adyacentes a la estenosis. Durante la dilatacion inicial se realizaron 2 ± 1,6 sesiones (2-18), obteniendose una buena respuesta en 18 pacientes (54%). Al final del seguimiento el 68% de los enfermos presentaba una situacion clinica satisfactoria. En el estudio comparativo aparecen como variables estadisticamente significativas para una buena respuesta al tratamiento: la ingesta accidental del alcali, el inicio del tratamiento en fase cronica, la ausencia de fenomenos inflamatorios adyacentes a la estenosis y un bajo numero de sesiones en la dilatacion inicial. Conclusiones En nuestra serie mas de la mitad de los pacientes con estenosis esofagica por ingesta de caustico de tipo alcali presenta una buena respuesta inicial. Aquellos con estenosis en fase aguda y que precisan mayor numero de sesiones en la dilatacion inicial tienen peor respuesta y la terapeutica endoscopica es menos eficaz.
