Anti-Jra (JR1) et anti-M (MNS1) responsables d’anémie fœtale sévère : un mécanisme d’action commun ?

Abstract

Dans la litterature, plusieurs cas d’anemies fœtales severes induits par des immunisations anti-M (MNS1) et anti-Jra (JR1) maternelles ont ete decrits et nous ont amenes a reprendre les observations d’atteinte fœtale au CNRHP ces dernieres annees. Afin d’etudier les caracteristiques biologiques maternelles et fœtales des grossesses compliquees d’immunisations anti-M et anti-Jra, nous avons extrait les cas d’anemie fœtale severe, suivis au CNRHP entre le 01/01/2007 et le 31/12/2016 et compare ceux par incompatibilite fœto-maternelle anti-M et anti-Jra a ceux par incompatibilite anti-D (RH1) et anti-K (KEL1). En 10 ans, 4 fœtus dont les meres avaient une immunisation anti-Jra et 2 fœtus dont les meres presentaient une immunisation anti-M ont beneficie de transfusion(s) in utero. Dans ces 6 cas, les titres des anticorps maternels etaient superieurs a 32 au moment de la 1re transfusion. L’anemie survenait apres 26 SA et etait associee a un faible processus hemolytique peripherique : faible positivite, voire negativite, du test direct a l’antiglobuline, faibles taux de bilirubines (
