Abstract

Introduction Nous rapportons le cas original d’une patiente ayant developpe un LPSNC apres avoir presente une lesion « inflammatoire » cerebrale operee 13 ans plus tot dans le meme siege. Observation Mme L., âgee de 72 ans presenta en mai 2020 des cephalees et des troubles de l’humeur avec des acces coleriques inhabituels, motivant la realisation d’une IRM cerebrale. Celle-ci retrouvait des lesions plurifocales parietales prenant le contraste en regard d’un ancien site operatoire. La patiente avait presente en 2007 un tableau associant cephalees, syndrome confusionnel et troubles sensitifs de l’hemicorps gauche, ayant fait decouvrir une lesion expansive de meme siege qui avait ete operee, et pour laquelle l’histologie avait conclu a une lesion inflammatoire. Dans ses autres antecedents on retrouve une meningite virale en 1959 et une reprise chirurgicale de plastie crânienne en septembre 2019. La patiente avait ensuite ete traitee par corticoides sans autre diagnostic retenu. La biopsie realisee en juillet 2020 retrouvait une infiltration tumorale lymphomateuse avec immunomarquage CD20 + KI-67 : 90 % ; hybridation in situ EBER : negative ; concluant a un lymphome B diffus a grandes cellules, de type centre non germinatif a localisation cerebrale. Le bilan systemique negatif confirmait le caractere primitif et la ponction lombaire etait normale (aucun element, proteinorachie a 0,53 g/L, glycorachie a 4,5 mmol/L). Discussion Cette observation souleve la question de la physiopathogenie, mal connue, des LPSNC de l’immunocompetent (transformation lymphocytaire maligne dans le cerveau ou metastase d’un lymphome systemique occulte ?). Elle permet de faire l’hypothese que certains LPSNC puissent survenir sur une lesion inflammatoire prealable (« lesion sentinelle »), d’autres cas ayant ete rapportes dans la litterature. Un element remarquable illustre ici est le long intervalle entre la « lesion sentinelle » et la transformation lymphomateuse si le lien suspecte est avere. Conclusion LPSNC chez une patiente ayant presente une lesion inflammatoire cerebrale d’etiologie indeterminee 13 ans auparavant soulevant des hypotheses concernant la physiopathogenie du lymphome cerebral chez le patient immunocompetent.
