Abstract

Resume Introduction La mononucleose infectieuse peut etre a l’origine de nombreuses complications. L’obstruction des voies aerodigestives par hyperplasie amygdalienne est la plus connue a l’extremite cephalique. La dacryocystite est plus rare et potentiellement grave. Observation Un enfant de six ans, avec une primo-infection mononucleosique identifiee depuis huit jours, a developpe une dacryocystite aigue. Elle s’est rapidement compliquee d’une cellulite orbitaire, malgre une antibiotherapie adaptee. Un drainage chirurgical en urgence a ete necessaire. Discussion La dacryocystite est une complication rare et meconnue de l’infection a Epstein-Barr virus. Son evolution aigue en cellulite orbitaire est possible et potentiellement grave. Sa physiopathologie est particuliere. Les patients sont initialement indemnes de stenoses chroniques et d’epiphora, expressions d’une obstruction aigue du sac lacrymal par hyperplasie lymphoide generalisee.
