779 Lymphome du sac lacrymal simulant une dacryocystite aiguë : à propos d’un cas

Abstract

Introduction Le lymphome du sac lacrymal est une pathologie rare pouvant se manifester par un tableau de dacryocystite aigue. Materiels et Methodes Un patient âge de 72 ans, ayant pour antecedent une dacryocystorhinostomie externe droite realisee 5 mois auparavant, s’est presente aux urgences avec un tableau de dacryocystite aigue droite. Apres instauration d’une antibiotherapie orale, l’inflammation s’est rapidement resolue. Un scanner orbitaire realise une semaine plus tard montrait un volumineux sac lacrymal droit. Une dacryocystorhinostomie a ete programmee pour drainer la dacryocele. Au cours de l’intervention, une masse non liquidienne a ete retrouvee au niveau de la loge lacrymale. La biopsie a permis de poser le diagnostic de lymphome du sac lacrymal. Discussion Les tumeurs du sac lacrymal sont rares. Parmi ces tumeurs, les lymphomes restent exceptionnels, representant 6 % de l’ensemble des tumeurs du sac lacrymal. Dans cette observation, les elements cliniques et radiologiques pre-operatoires n’ont pas permis d’evoquer le diagnostic de tumeur du sac lacrymal. Conclusion Les tumeurs du sac lacrymal sont de diagnostic difficile dans les stades precoces, mais aussi parfois dans les stades avances. Les signes cliniques aspecifiques peuvent conduire a un diagnostic errone et une biopsie doit etre realisee en cas de doute.
