Fatores preditivos para marcha na fratura transtrocanteriana do fêmur

Abstract

OBJETIVO: Identificar variáveis capazes de predizer a qualidade de marcha dos pacientes com fratura transtrocanteriana após o tratamento. MATERIAIS E MÉTODOS: Pacientes internados com o diagnóstico de fratura transtrocanteriana foram selecionados em um período compreendido entre setembro/05 e agosto/06 e acompanhados por seis meses após a data do trauma. Realizou-se estudo prospectivo observacional, sendo avaliada a qualidade de marcha 3 e 6 meses pós fratura de 31 pacientes, 13 masculinos e 18 femininos. A média de idade foi de 76±2,7 anos. RESULTADOS: Houve sete (22,6%) óbitos durante o seguimento. Os pacientes com fraturas associadas ou com mais de quatro comorbidades apresentaram pior qualidade de marcha após seis meses. Pacientes sem complicações ortopédicas ou que adquiriram carga parcial anterior há 30 dias mostraram-se com melhor desempenho. CONCLUSÃO: A quantificação dos índices preditivos de marcha permite a proposição de novas abordagens de tratamento coerentes com as diferentes realidades de cada grupo de pacientes.
