THE CLINICAL SIGNIFICANCE OF ACCESSORY BILE DUCT: EVALUATION WITH PTC AND ERCP

Abstract

最近5年間に当科で術前に直接胆道造影 (PTCあるいはERCP) を行った肝胆道系疾患450例を検索し, 外科的に問題となる副胆管は21例 (4.7%) にみられた.副胆管を外科的な立場から, その開口部位によりType I: 副胆管が肝より直接胆のうに開口, TypeII: 副胆管が胆のう管に, あるいは胆のう管が副胆管に開口, TypeIII: 副胆管が総肝管に開口するもので右副胆管をIIIa, 左副胆管をIIIb, TypeIV: 副胆管が総胆管に開口するもので右副胆管をIVa, 左副胆管をlvbの4つの型に分類し, Type別に症例を供覧するとともに, 集計しえた体邦報告例と自験例を中心に, その頻度, 形態および術中副胆管損傷の危険性について検討した.
