Abstract

Zur Langzeitsedierung im Kindesalter im Rahmen der Intensivmedizin hat sich die Kombination von Benzodiazepinen und kurzwirksamen Opioiden bewahrt. Meist wird hier die Kombination Midazolam/Alfentanil oder Midazolam/Fentanyl benutzt. Besteht jedoch die Notwendigkeit, uber einen langeren Zeitraum (>3–4 Tage) eine kontrollierte Beatmung durchzufuhren, beobachten wir unter den angegebenen Schemata haufig Toleranzphanomene, die entsprechende Dosiserhohungen und zusatzliche Gaben von Sedativa und Analgetika notwendig machen. Daruber hinaus entwickeln viele dieser Kinder wahrend der Phase der Entwohnung vom Respirator Entzugssymptome wie Unruhe, Halluzinationen und ausgepragte Bewegungsstorungen. Die guten Erfahrungen mit dem α2-Agonisten Clonidin als Supplement zur Analgesie und bei der Entzugsbehandlung im Erwachsenenalter sowie einzelne Erfahrungen bei Kindern nahmen wir zum Anlas, den Stellenwert dieser Substanz im Rahmen der Langzeitsedierung zu evaluieren [1–4].
