Artrodesis, resección y coaptación de cadera

Abstract

El rendimiento y el aumento de la supervivencia de las artroplastias totales de cadera han provocado una disminucion considerable de la realizacion de artrodesis, reseccion y coaptacion en una cadera nativa. Sin embargo, estas posibilidades terapeuticas deben plantearse en pacientes jovenes en quienes el pronostico de las protesis de cadera es incierto a muy largo plazo. Las indicaciones principales de estas tecnicas son: los fracasos ultimos postraumaticos o de artroplastias en los que no es posible recurrir a una revision protesica por motivos mecanicos (destruccion articular importante, deficit del capital oseo, trastornos neurologicos o musculares), medicos (riesgo anestesico importante) o infecciosos. Por ultimo, esta cirugia sigue estando indicada en los pacientes en quienes la artroplastia tiene una contraindicacion relativa o una relacion beneficio-riesgo desfavorable a la artroplastia: se trata de los pacientes neuroortopedicos en quienes la reseccion o la coaptacion de cadera permite liberar un sector de movilidad, los pacientes con riesgo septico elevado o aquellos con una demanda funcional reducida.
