Diagnostic rapide du paludisme par la recherche d'antigènes circulants

Abstract

Resume Les methodes de diagnostic biologique du paludisme doivent avoir une bonne sensibilite, une bonne specificite et etre simples et rapides a executer et a interpreter. Depuis quelques annees, ont ete mis au point des tests rapides, bases sur la detection de Plasmodium par immuno-capture d'antigenes circulants. Les tests ParaSight™-F et ICT Malaria Pf™ decelent la proteine riche en histidine (HRP-II) ; le test OptiMAL™ recherche l'enzyme lactate-deshydrogenase du parasite (pLDH). Ces techniques permettent de detecter environ 50–100 parasites/μL de sang. Leur sensibilite et leur specificite sont voisines de 90 %. Le ParaSight™-F et l'ICT Malaria Pf™, ne depistent que les cas d'infection par Plasmodium falciparum, tandis que l'OptiMAL™ permet un diagnostic de l'espece plasmodiale. Ce dernier test permet egalement de suivre l'efficacite d'un traitement alors que pour les deux premiers, se pose le probleme de la persistance de l'antigene HRP-II circulant apres disparition de la parasitemie.
