P54. RADIOPROTECÇÃO NAS TERAPÊUTICAS DE CARCINOMA DA TIROIDEIA COM IODO-131 – A EXPERIÊNCIA DO HOSPITAL DA LUZ

Abstract

Resultados: Foram estudados 19 doentes, 68,4% sexo feminino, idade media 15,9 ± 3,4 anos (9-20), seguimento medio 5,9 ± 4,6 anos (1-16). A tumefaccao cervical foi a forma de apresentacao mais frequente (11 doentes), 2 com adenopatia cervical, 3 diagnosticados incidentalmente. Nenhum com antecedentes de irradiacao cervical, 9 (52,3%) com historia familiar de patologia tiroideia, 5 (29,4%) com tiroidite autoimune. Ecograficamente 82,3% apresentava nodulos solidos iso ou hipogenicos, 17,8% com microcalcificacoes, tamanho medio 2,3 cm (0,5-4,1). Citologia (n = 15): 2 (13,3%) coloide, 2 (13,3%) tumor folicular, 4 (26,7%) sugestiva de carcinoma papilar, 7 (46,7%) carcinoma papilar. Dezoito doentes submetidos a tiroidectomia total, 3 com esvaziamento cervical, um realizou lobectomia. AP: todos carcinomas papilares (3 variantes de pior prognostico), 2 (10,6%) microcarcinomas, 6 (31,6%) multifocais, 4 (21,1%) com invasao vascular, 6 (31,6%) com metastizacao ganglionar. Dezasseis (82,4%) submetidos a terapeutica ablativa com 131I (em media 3,4 meses apos a cirurgia). Todos sob terapeutica supressiva com levotiroxina. Seis doentes foram submetidos a terapeutica com iodo radioactivo (1 a 4 tratamentos), dose media cumulativa de 198mCi. Uma doente foi submetida a lobectomia pulmonar por metastizacao pulmonar bilateral. Ultima avaliacao: 16 doentes livres de doenca, 2 com tiroglobulina positiva e imagem negativa, 1 com metastizacao pulmonar com boa resposta ao tratamento. Conclusao: Nesta serie, todos os casos corresponderam a carcinoma diferenciado (15,7% com variantes de pior prognostico). Metastizacao ganglionar cervical e a distância foi detectada em 31% e 5%, respectivamente. Estes resultados evidenciaram o bom prognostico desta patologia mas nao a maior agressividade descrita habitualmente nesta populacao.
