Manejo actual del cáncer microinvasor del cérvis: Hospital de San José

Abstract

En el mundo el cáncer de cuello uterino ocupa el segundo lugar en incidencia después del de mama y cerca de tres cuartas partes de los casos ocurren en países en desarrollo. Durante las últimas dos décadas la incidencia en los Estados Unidos ha disminuido en casi un tercio gracias al advenimiento y la aplicación de programas de cribado. Desde hace tiempo se ha establecido la asociación entre el cáncer de cuello uterino y un agente causal transmitido en la relación sexual llamado virus de papiloma humano, que en la patogénesis tiene una prevalencia del ADN del VPH en más del 90% de las lesiones preinvasoras e invasoras. El carcinoma de cérvix se diagnostica en el 21% en mujeres jóvenes con vida sexual activa y que aun no han completado su deseo de paridad. Este artículo revisa las estrategias actuales en el manejo de los pacientes con carcinoma microinvasor de cérvix y considera los mecanismos de acción terapéutica. Abreviaturas: VPH, virus del papiloma humano; NIC, neoplasia intracervical; LIE, lesión intraepitelial.
