Die Korporoplastik nach Nesbit — Eigene Ergebnisse zwischen 1970 und 1987

Abstract

Die Ursachen der Penisdeviation sind vielfaltig, die Korporoplastik nach Nesbit ist die Methode der Wahl bei allen Formen der Corpus cavernosum-Dysplasien sowie mitunter der Induratio penis plastica oder posttraumatischen Narbenbildungen. Unser eigenes Untersuchungsgut (Tabelle 1) umfast 11 Patienten. Der Grad der Penisdeviation lag mehrheitlich zwischen 45 und 60 Grad mit einem Krummungsmaximum im mittleren Penisschaftdrit-tel. Bei 7 Patienten verlief die Abknickung nach ventral, bei je 2 Patienten nach dorsal oder lateral. Als pathomorphologisches Korrelat fand sich intraoperativ bei 10 Patienten eine Corpus caverno-sum-Dysplasie, in 3 Fallen zusatzlich Chordareste, zweimal eine Hypospadia glandis ohne Corpus spongiosum-Dysplasie. Bei einem Patienten (Abb. 1–4) handelt es sich um eine ausgepragte Form der Induratio penis plastica. Die histologischen Befunde waren unspezifisch, in allen Fallen zeigte sich lediglich Bindegewebe. Bei ventralen Ab-knickungen war meist das Abheben des dorsalen Gefasnervenbundels zwingend. In einem Fall (Abb. 1–4) muste zusatzlich die Urethra von den Corpora cavernosa ausgelost werden. Intra- und postoperative Komplikationen sind selten. Das intraoperative Aufrichtungsergebnis korreliert in allen Fallen mit den postoperativen Spatbefunden. Bei Nachuntersuchung war der Grad der Abweichung kleiner 10 Grad. Eine Dyspareunie war nicht mehr gegeben.
