Abstract

La anquilosis dental se produce por pérdida de continuidad del ligamento periodontal permitiendo la unión del hueso alveolar con el cemento. Es más prevalente en la dentición primaria (1,3 y 8,9%), sin diferencia entre sexos, afectando mayormente primeros molares primarios inferiores. Impide el proceso eruptivo normal y el crecimiento vertical óseo del diente anquilosado, quedando por debajo del plano oclusal ocasionado pérdida de perímetro del arco, inclinación de dientes adyacentes, pérdida de espacio y alteraciones de la masticación. Las anomalías de desarrollo dental pueden presentarse de forma conjunta, combinando presencia de dientes anquilosados y dientes supernumerarios. Objetivo: Presentar el caso clínico, diagnóstico y plan del tratamiento de molares primarios severamente anquilosados y mesiodens superior. Presentación de caso: Paciente femenina de 7 años de edad, con diastema entre 11 y 21 por inserción baja del frenillo labial y de supernumerario tipo mesiodens cónico invertido. Presenta anquilosis severa de 65 y 85, posición apical de gérmenes 25 y 45, con defecto alveolar vertical e inclinación mesial de 26 y 46 y acortamiento de perímetro de arco. Se realizan imágenes volumétricas para determinar posición del supernumerario y grado de anquilosis de dientes primarios. Se indica bajo anestesia general en ambiente hospitalario odontectomía de supernumerario y dientes primarios anquilosados 65 y 85. Se planifica tratamiento de ortodoncia interceptiva con tornillos distalizadores para 46 y 26. Conclusión: el tratamiento oportuno de los dientes anquilosados es fundamental para evitar consecuencias negativas en el crecimiento y desarrollo de los maxilares, evitar maloclusiones e impactación de gérmenes sucesores.
