O39 Taux circulants d’adrénomédulline, risque coronarien et mortalité chez des sujets diabétiques de type 2

Abstract

Introduction L'adrenomedulline est un peptide ubiquitaire, qui semble etre un mediateur multifonctionnel dans la proliferation cellulaire, l'angiogenese, la permeabilite endotheliale et la regulation hormonale. L'adrenomedulline a un effet vasodilatateur et son expression est accrue lors de l'hypoxie. Nous avons evalue, dans deux cohortes prospectives de diabetiques de type 2, l'association entre MR-proADM a l'inclusion et le risque d'evenements coronariens (infarctus du myocarde, revascularisation coronarienne, mort subite) et la mortalite totale, pendant le suivi. Patients et methodes MR-proADM plasmatique a ete mesure chez 2 962 sujets de la cohorte DIABHYCAR (âge 66±8 ans, 2 170 hommes/792 femmes, suivi median de 4,7 ans) et 1 351 sujets de la cohorte SURDIAGENE (âge : 65±11 ans, 776 hommes/575 femmes, suivi median de 5,3 ans). Resultats Dans DIABHYCAR, la mortalite totale par terciles de MRproADM a ete 10,6 % (T1), 14,1 % (T2) et 19,5 % (T3) : HR 1,6, 95 % C.I. 1,2–2,2 (p=0,0002, T3 vs T1, ajuste sur l'âge et sexe) et dans SURDIAGENE, de 10,9 % (T1), 18,4 % (T2) et 41,5 % (T3) : HR 3,2, 95 % C.I. 2,3–4,5 (p Conclusion Nos donnees etablissent que MR-proADM plasmatique est un facteur independant de deces chez les patients diabetiques de type 2.
