Tratamiento de la espondilolistesis en el niño

Abstract

La espondilolisis y la espondilolistesis son anomalias frecuentes en los ninos, cuyo tratamiento es aun objeto de discusion tanto desde el punto de vista etiologico como terapeutico. La evolucion de estas enfermedades es igualmente especifica en los ninos, con riesgo de agravarse durante la fase de crecimiento y con estabilizacion potencial de las lesiones a continuacion. Deben distinguirse dos entidades: las formas con escaso desplazamiento y las formas de grado alto. El tratamiento de las espondilolistesis con escaso desplazamiento depende principalmente de la sintomatologia: puede ir desde el tratamiento funcional hasta el tratamiento quirurgico mediante reconstruccion istmica o artrodesis lumbosacra. Los desplazamientos de grado alto son, en cambio, formas inestables cuyo riesgo neurologico justifica una intervencion quirurgica. Dependiendo de la cifosis lumbosacra, se diferencian las espondilolistesis con sacro horizontal o vertical. Su tratamiento es complejo y se han descrito muchas alternativas terapeuticas. En este articulo se trata una tecnica especifica de artrodesis circunferencial por via transacra.
