Abstract

症例は35歳,女性.中枢性尿崩症で発症し,発熱と頭痛,更に無月経を認め,頭部MRIにてトルコ鞍から鞍上部へ進展する腫瘤性病変を認めた.当院脳外科にて経蝶形骨洞的手術による生検を行いリンパ球性漏斗神経葉炎の診断が得られた.内分泌学的検査では視床下部性下垂体機能低下症を認め,ホルモン補充療法が開始された.その後,腫瘤性病変は自然退縮し,部分的な下垂体前葉系の機能の改善が認められたが, 4年後の機能評価にても完全な改善は認められず,補充療法を中止できない状態である.
