El minihemocultivo como herramienta diagnóstica en sepsis neonatal

Abstract

Resumen Objetivo Disenar un minihemocultivo usando de 200 a 300 μl de sangre y comparar con el hemocultivo de referencia BACTEC peds plus/F™ BD. Metodos Se prepararon dos lotes de minihemocultivos HIM-1 y HIM-3 con diferente composicion. Se les hizo el control de calidad que se lleva a cabo en el BACTEC, que es el hemocultivo automatizado de referencia. Se estudiaron 98 muestras de pacientes de la unidad de cuidados intensivos neonatales con probable sepsis y se estratificaron en dos grupos; en el primero, 48 muestras se inocularon en el HIM-1 y en el BACTEC pediatrico, y en el segundo, 50 muestras en el HIM-3 y en BACTEC pediatrico. Resultados HIM-1 y HIM-3 permitieron el crecimiento de los microorganismos: E. coli, K. pneumoniae, P. aeruginosa y Enterococcus spp. hasta 102 UFC/ml (que es la concentracion minima de recuperacion del hemocultivo de referencia). Los minihemocultivos no favorecieron el crecimiento de S. epidermidis, S. aureus y S. pneumoniae de 102 UFC/ml. Para gramnegativos se obtuvo una sensibilidad del 80%, especificidad del 96%, valor predictivo positivo del 80% y valor predictivo negativo del 96%. Conclusiones HIM-1 y HIM-3, probablemente serian adecuados para el diagnostico de sepsis neonatal temprana y en paises en desarrollo, principalmente por gramnegativos, pero no para sepsis tardia, principalmente por estafilococos. En comparacion con BACTEC, ahorran gran cantidad de muestra sanguinea.
