
In brief A high school football player suffered an acute lateral lower leg compartment syndrome following an apparent peroneal muscle strain. Early diagnosis was made on the basis of worsening symptoms and signs and by intracompartment pressure readings. Prompt fasciotomy and removal of necrosed tissue, followed by casting and then air splinting, allowed a full recovery and return to play.
