Abstract

Tra i 18 siti di interesse nazionale per le bonifiche (SIN) analizzati ce ne sono alcuni con esposizione ambientale unica e con un unico inquinante (per esempio Biancavilla, fluoro-edenite) ma, nella maggior parte dei casi, si è in presenza di molteplici ed eterogenee sorgenti emissive. Per questo la presenza di una evidenza a priori di associazione con le fonti di emissione/rilascio del SIN è di aiuto nel riferire il profilo di salute della popolazione residente a specifiche esposizioni ambientali. Questa coerenza con l’evidenza a priori è stata evidenziata in diverse circostanze, come per esempio nel SIN Fidenza per il tumore dello stomaco (eccesso di incidenza in entrambi i generi); nel SIN Laguna di Grado e Marano per il tumore dello stomaco (eccessi di mortalità, incidenza e ricoverati tra le donne); nei SIN di Brescia-Caffaro, Milazzo, Terni-Papigno per le malattie respiratorie, con eccesso di ricoverati; nel SIN di Brescia-Caffaro l’eccesso di incidenza e di ricoverati per linfomi non-Hodgkin e melanoma (in entrambi i generi) e per tumore della mammella tra le donne. Per la valutazione del profilo di salute della popolazione risulta importante anche la coerenza fra diversi indicatori (per esempio eccessi nella mortalità, incidenza e ricoverati per il tumore della vescica, negli uomini, a Porto Torres; per l’apparato urinario nel SIN Basso bacino del fiume Chienti), così come la coerenza dello stesso indicatore in entrambi i generi (per esempio l’eccesso di incidenza del totale dei tumori a Bolzano, Porto Torres, Venezia, Litorale Domizio Flegreo, Priolo, Taranto; l’eccesso di mortalità generale a Taranto, Litorale Domizio Flegreo, Trieste). I singoli eccessi emersi nei diversi SIN da una parte devono essere valutati con attenzione, come stimolo per approfondimenti di tipo analitico rivolti a confermare e precisare evidenze di associazioni causali con esposizioni ambientali specifiche, dall’altra però possono considerarsi già utilizzabili per indicare e modulare cogenti misure di prevenzione primaria.

Of the 18 National Priority Contaminated Sites (NPCSs) analysed in this Report, some have a single source of environmental contamination (such as fluoro-edenite in Biancavilla). In most cases, however, we are looking at multiple heterogeneous sources of contamination. In this respect, the a priori causal evaluation of the association between diseases and environmental exposures in NPCSs, based on epidemiological evidence, can help trace the health impact back to specific types of environmental exposure. There are several cases in which the project’s findings have been consistent with a priori evidence: stomach cancer (both genders, excess cancer incidence) in the Fidenza NPCS; stomach cancer (women, excess mortality, cancer incidence and hospital discharges) in the Laguna di Grado e Marano NPCS; excess hospitalisation from respiratory diseases in Brescia-Caffaro, Milazzo and Terni Papigno NPCSs; excesses for non-Hodgkin lymphomas and melanoma (incidence and hospitalisation in men and w
