Dificultad respiratoria aguda

Abstract

La dificultad respiratoria es un motivo de consulta frecuente en Urgencias pediátricas. La primera medida es la evaluación del paciente mediante el triángulo de evaluación pediátrica (TEP), monitorización y estabilización con administración de oxigenoterapia. La historia y exploración física son suficientes para la orientación diagnóstica en la mayoría de los casos. La etiología más frecuente es la respiratoria, pero se deben barajar la metabólica o cardiovascular. La laringitis aguda se define como: obstrucción de vía aérea superior con la característica tos metálica y estridor inspiratorio. Según su gravedad, puede ser necesario tratamiento con corticoides (orales y nebulizados) o adrenalina nebulizada. La bronquiolitis es la primera infección del tracto respiratorio inferior en lactantes menores de 24 meses. Su diagnóstico es clínico y, según su gravedad, se instaura un tratamiento de soporte, sin que exista un fármaco que se haya demostrado útil en el curso de la enfermedad. La crisis asmática se presenta como: tos, disnea y opresión torácica, y la auscultación típica son las sibilancias. Su tratamiento consiste en broncodilatadores y, en algunos casos, corticoides sistémicos. La neumonía es la infección del parénquima pulmonar. El diagnóstico es clínico y se confirma por prueba de imagen. Según su etiología, puede ser necesaria la antibioticoterapia sistémica.
