Abstract

Las fracturas de la base del 5S metatarsiano constituyen un motivo de consulta frecuente en los Servicios de Urgencias de Traumatología. Por lo general, el paciente refiere un mal gesto del tobillo o del pie durante la deambulación o al realizar una practica deportiva. Un mejor conocimiento de la anatomía patológica de la lesión y de la vascularización del 5s metatarsiano, ha permitido clasificar estas fracturas, que se conocían genéricamente como fracturas de Jones, en tres tipos: tuberositarias, de Jones y por sobrecarga. Como señalan los autores, esta clasificación es interesante, ya que permite establecer un pronóstico lesional. El tratamiento habitual es conservador, mediante la colocación de una botina de yeso, aún cuando algunos autores limitan la inmovilización a un vendaje elástico adhesivo. La cirugía queda reservada para los retardos de consolidación, las pseúdoartrosis y algunos casos en que interesa un tiempo de curación lo más corto posible, como son los deportistas de élite. Finalmente, recordar que en ocasiones debemos realizar un diagnóstico diferencial con los huesos supernumerarios que podemos encontrar en esta zona, con la enfermedad de Iselin y con el centro de osificación secundario de la apófisis del 5S metatarsiano.
