Taponamiento cardiaco como presentación del cáncer de origen desconocido

Abstract

Fundamento: el derrame pericárdico y el taponamiento cardíaco son complicaciones conocidas de las enfermedades malignas aunque raramente son su forma de manifestación inicial. Caso clínico: se presenta el caso de una mujer de 52 años que acudió a urgencias por un cuadro de disnea, astenia y dolor en punta de costado izquierdo de 4-5 días de evolución que se etiquetó en un principio como neumonía alveolointersticial. A las 24 horas de ingreso presentó taponamiento cardíaco que requirió actuación inmediata con drenaje pericárdico. El análisis del líquido mostró células malignas, compatibles con adenocarcinoma. El estudio efectuado no logró hallar el tumor primario y, un mes más tarde, la paciente murió en su domicilio de forma súbita sin que se practicara el estudio necrópsico. Discusión: se aporta un nuevo caso de taponamiento cardíaco como presentación de cáncer de origen desconocido, hecho que se describe en la bibliografía de forma muy excepcional.
