Eficacia de la sustitución de ciclosporina A por tacrolimus en la disinución del agrandamiento gingival en los pacientes trasplantados renales

Abstract

Objetivos: este estudio pretende valorar la eficacia del tratamiento periodontal junto con tacrolimus en la supresión del agrandamiento gingival (AG) y el efecto que produce la supresión de ciclosporina A por tacrolimus en el AG. Diseño del estudio: dieciséis pacientes trasplantados renales con una media de edad de 52 años, cuya función renal es estable y que siguen tratamiento con ciclosporina A, fueron asignados de forma randomizada a 2 grupos. En el grupo casos se dieron instrucciones de higiene bucal y se efectuó tratamiento periodontal, mientras que en el grupo control sólo se dieron instrucciones de higiene bucal. Tras la 1ª visita y el cambio de medicación de ciclosporina a tacrolimus en ambos grupos, se hicieron revisiones clínicas periódicas durante 3 meses para valorar la evolución de su AG. Resultados: en todos los pacientes se observó una disminución progresiva del agrandamiento gingival, no existiendo diferencias estadísticamente significativas entre ambos grupos (p>0,05). Es en el primer mes tras el cambio de medicación cuando hay una mayor disminución del agrandamiento gingival. Conclusiones: no se ha observado una mayor eficacia del tratamiento periodontal en combinación con el tacrolimus en la reducción del AG. El tacrolimus es una alternativa a la ciclosporina cuando se quiere evitar el AG en los pacientes trasplantados renales.
