Abstract

Se presenta la casuística sobre 24 casos de fracturas graves de cuello de húmero. Se clasifican estas fracturas en cinco grupos para establecer la pauta terapéutica adecuada en cada grupo. 1. Fracturas luxaciones. 2. Fracturas conminutas. 3. Fracturas inestables. 4. Desprendimientos epifisarios. 5. Pacientes polifracturados o politraumatizados. Las fracturas luxaciones se consideran, en pacientes que no han llegado a la senectud siempre quirúrgicas. Si se considera la cabeza viable, intentamos, una vez reducida, la fijación con grapas. Si se considera inviable, es decir, abocada a la necrosis, practicamos la extirpación y reconstrucción tenoplástica según técnica de L. Jones. En las fracturas conminutas la pauta terapéutica es fundamentalmente la misma que en el grupo anterior. Las fracturas inestables se han fijado, una vez reducidas, mediante un enclavado en haz, percutáneo, desde la fosita olecraniana según técnica de Hacketal. El grupo de los desprendimientos epifisarios, una vez reducido, ha sido tratado mediante escayolado toracobraquial en posición de guardia urbano. Finalmente, en el grupo de pacientes polifracturados usamos el enclavado en haz para evitar cualquier tipo de vendaje y facilitar la movilización del paciente y los cuidados de enfermería. Este método permite en fracturas del húmero a distintos niveles contener los distintos fragmentos mediante un solo acto terapéutico. En pacientes poli traumatizados, con lesiones abdominales o torácicas que impiden la colocación de una férula de abducción o yeso toracobraquial, empleamos la tracción suspensión. También serían estos pacientes susceptibles de tratar mediante el enclavado en haz. Se presenta un caso ejemplo de cada tipo de tratamiento expuesto.
