Abstract

L'identificazione precoce della carenza di cobalamina (tB12) è importante in soggetti asintomatici e con livelli di tB12 normali, ma a rischio di sviluppare deficit vitaminico in seguito ad un particolare stato fisiologico o stile di vita. La carenza, anche latente, di cobalamina può comportare, tra l'altro, iperomocisteinemia. Per valutare l'utilità clinico-diagnostica della determinazione dell'Olotranscobalamina (Holo-TC), frazione attiva della tB12, è stato analizzato lo status della cobalamina in 227 soggetti suddivisi in 5 gruppi (anziani, forti fumatori, obesi, donne in terapia estroprogestinica, vegani). Per ciascun soggetto veniva determinato il metabolismo dell'omocisteina e delle vitamine metabolicamente correlate (tB12 e folato). In ciascun gruppo erano presenti soggetti con moderata iperomocisteinemia, in parte dovuta al deficit di cobalamina (anche con folato nella norma). Il deficit di tB12 (<164pM) era presente in circa il 50% dei soggetti carenti di HoloTC (<40pM). La debole correlazione tra tB12 e Holo-TC (R2 =0,39) indicava che la carenza della tB12 e della sua frazione biologicamente attiva non sempre coincidono. La determinazione di Holo-TC può quindi evidenziare una carenza di cobalamina anche in presenza di valori di tB12 normali. Il nostro studio suggerisce l'utilità di una sua valutazione soprattutto in soggetti asintomatici.
