Abstract

La leucemia cutis è caratterizzata da un infiltrato cutaneo di leucociti neoplastici della serie melodie linfoide e si può manifestare in forme acute o croniche di leucemia. Un uomo di 75 anni, seguito da alcuni mesi in Ematologia per una forma mielodisplastica cronica di leucemia mieli-monocitica, presentava da 1 settimana un'eruzione di asintomatica diffusa al tronco, costituita da numerose papule monomorfe di colore rosso scuro e consistenza aumentata. L'esame istologico e l'immunoistochimica documentarono l'interessamento cutaneo di una forma blastica acuta di leucemia mielo-monocitica con contemporaneo interessamento midollare e del sangue periferico, a prognosi rapidamente infausta nel nostro caso. Negli adulti in corso di leucemie croniche o sindromi mielodisplastiche, l'interessamento cutaneo è generalmente associato a trasformazione blastica e a progressione neoplastica e pertanto rappresenta un segno prognostico sfavorevole.

Leukemia cutis is defined as cutaneous infiltration by myeloid or lymphoid neoplastic leukocytes and occurs both in chronic and acute leukemia subtype. A 75-old-year man was in haematological follow-up since few months for a chronic myelodysplatic disorder defined as chronic myelo-monocytic leukemia. He presented a diffuse asymptomatic eruption of many monomorphic dark-red papules of increased texture. The histopathological examination and the immunohistochemistry study were consistent with a cutaneous infiltration of a blastic acute myelo-monocytic leukemia, contemporary invading peripheral blood and bone marrow and leading rapidly to death our patient. During chronic leukemia and myelodysplatic disorders of adults, the onset of leukemia cutis is often associated with blastic transformation and neoplastic progression, representing an unfavourable prognostic sign.
