Abstract

Negli anni 70 e 80 del secolo appena concluso le osteotomie d’anca ebbero largo spazio nel trattamento della coxartrosi displasica. L’indicazione agli interventi osteotomici era posta prevalentemente in pazienti giovani con importanti alterazioni scheletriche, con dolore, ma in presenza di articolarità sostanzialmente conservata. Oggi sicuramente le indicazioni alle osteotomie sia di bacino, che di femore, sono ridotte, soprattutto in considerazione della notevole aspettativa di sopravvivenza di una moderna protesi. Rimane comunque il fatto che, almeno nella nostra casistica, l’osteotomia di Chiari è stata eseguita in 1124 pazienti. Di questi il 134 casi (12%) ha dovuto, sempre in nostre mani, essere protesizzato. I problemi nella protesizzazione del cotile dopo osteotomia di Chiari, sono prevalentemente legati a fattori quali: il notevole assottigliamento del fondo del cotile, la scopertura sia anteriore che posteriore, la grande ampiezza del cotile (cotile magno). In queste situazioni cotili avvitati sia a tronco di cono che emisferici, in lega di titanio, trovano ideale applicazione. Spesso però non è possibile raggiungere una sufficiente stabilità della componente acetabolare senza il ricorso a sistemi da revisione. In casi selezionati siamo ricorsi, negli ultimi anni, all’impianto di cotili particolari (Octopus, McMinn) con risultati molto soddisfacenti. Dal 2001 abbiamo iniziato a utilizzare un modulo di navigazione computerizzato TAC guidato per l’inserimento della coppa protesica. Al momento non vi sono invece sistemi che possano integrare l’esperienza del chirurgo nell’impianto della componente femorale, quando la pregressa osteotomia, che abbia comportato una modifica del valgismo, dell’estensione e della rotazione del collo, abbia comportato il sovvertimento dei normali reperi anatomici. L’utilizzo di protesi coniche (tipo la Conus di Wagner) rende meno difficile l’impianto, ma non risolve per sè il delicato problema di un corretto orientamento e di una esatta ricostruzione del centro di rotazione della neoarticolazione.

During the last decades of the past century osteotomic surgery of the pelvis and proximal femur has been frequently performed in the treatment of hip dysplasia, mainly due to serious concern related to life expectancy of prosthetic implants. Pelvic osteotomies (such as the Chiari's procedure) as well as femur osteotomies (such as valgus-extension according to Bombelli) were performed in relatively young patients with mild-moderate degenerative articular changes. Since the main goal of such procedures was to delay the progression of the illness and relieve the symptoms as well, we have observed that in our case series of 1124 patients submitted to Chiari osteotomy, 134 (12%) of those patients underwent a subsequent total hip replacement. The implant of the prostheses in such population is definitely more technically demanding due to the cotyle modification (bone stock, shape, anterior and posterior deficient contour, abnormal centre of rotation). Recently computer assisted surgery might help at least the acetabular implant even if the experience of the chief surgeon cannot be undervalued.
