Abstract

In età adulta, circa il 20% delle donne soffre di emicrania, disturbo tipicamente femminile che esordisce di solito dopo la pubertà, spesso in coincidenza del menarca. Questa forma di cefalea primaria provoca nelle pazienti affette una notevole disabilità e un marcato deterioramento della qualità di vita. L’emicrania può essere considerata un fenomeno neurobiologico complesso, a componente neurovascolare, probabilmente basato su una predisposizione genetica e spesso a distribuzione familiare, legato ad anomalie transitorie del funzionamento delle cellule nervose senza apparenti alterazioni strutturali del sistema nervoso. Le basi neurofisiologiche dell’emicrania sono poco conosciute; sono comunque coinvolti diversi neurotrasmettitori, proteine della membrana neuronale e mediatori dell’infiammazione. In molti casi, l’attivazione dei meccanismi responsabili del dolore è condizionata da fattori scatenanti riconoscibili. Nella donna, i fattori scatenanti sono spesso rappresentati da modificazioni ormonali o metaboliche correlate con le varie fasi della vita riproduttiva: ciclo mestruale, assunzione di contraccettivi orali, climaterio, menopausa, assunzione di terapia sostitutiva ormonale. In questo articolo, si prenderanno in considerazione proprio i legami dell’emicrania con problematiche di tipo ginecologico, e si approfondiranno gli aspetti clinici che tali legami comportano.
