Abstract

La grossesse ectopique (GE) touche 1 à 2 % des grossesses et entraîne un risque de morbi- mortalité maternelle important. L’approche thérapeutique par méthotrexate (MTX) est bien établie. Cependant, certains points, tels que le seuil d’hCG sérique acceptable pour envisager un traitement médicamenteux ou l’intérêt de l’utilisation d’un score prédictif pour guider la décision thérapeutique restent débattus. Cette étude rétrospective multicentrique a permis d’évaluer l’efficacité du MTX pour le traitement des GE au sein du Département de Gynécologie-Obstétrique de Liège ainsi que l’impact de deux scores prédictifs. Au sein d’une cohorte de 256 patientes, 206 (80,5 %) ont pu être traitées uniquement grâce au MTX. Trois facteurs prédictifs de succès ont pu être identifiés : le type de GE, un taux initial d’hCG inférieur à 1.500 UI/L et l’absence d’activité cardiaque fœtale. L’utilisation de ces deux scores au sein de cette cohorte ne semble pas améliorer significativement le taux de succès.

Ectopic pregnancy (EP) affects 1-2 % of all pregnancies and is a significant risk of maternal morbidity and mortality. Treatment with methotrexate (MTX) is well established. However, certain aspects, such as the acceptable serum hCG threshold for considering medical treatment or the usefulness of predictive scores to guide therapeutic decisions, remain controversial. The aim of this multicenter retrospective study was to evaluate the efficacy of MTX in the treatment of EPs at the Gynaecology Department in Liège and the impact of two predictive scores. Of 256 patients included, 206 (80.5 %) were successfully treated with MTX alone. Three predictive factors for success were identified: the type of EP, an initial hCG level below 1500 IU/L and the absence of fetal cardiac activity. The use of these two scores in this cohort did not significantly improve the success rate.
