Abstract

[Resumen] La displasia broncopulmonar (DBP) es la principal patología respiratoria crónica asociada a la prematuridad, y su predicción y diagnóstico precoz continúa siendo un desafío en neonatología. En este estudio se incluyeron recién nacidos ≤ 32 semanas y ≤ 1500 gramos, con el objetivo de analizar haplogrupos mitocondriales y niveles de ADN mitocondrial circulante libre (ccf-mtDNA), como predictores del desarrollo de DBP o muerte. La cohorte estuvo compuesta por 107 neonatos: 44 (41,1%) desarrollaron DBP/éxitus (grupo de casos), y 63 (58,9%) sobrevivieron sin DBP (grupo control). El haplogrupo H fue el más prevalente (35,5%), seguido de U (13,1%), así como los clústeres SHV/H/V y U/K (39,3% y 19,6%, respectivamente), sin diferencias en la distribución entre grupos. Los niveles medios de ccf-mtDNA fueron 77.603 ± 124.974 U/μl, superiores en el grupo de casos frente a controles (87.443 ± 164.365 vs. 69.168 ± 78.727 U/μl), sin significación estadística. Las variables que se asociaron de forma independiente con el desenlace fueron la edad gestacional, el peso al nacer, la fracción inspiratoria de oxígeno máxima en sala de partos, las horas de ventilación mecánica y el uso de corticoides postnatales. Este es el primer estudio que analiza simultáneamente haplogrupos mitocondriales y ccf-mtDNA en grandes prematuros. Aunque no se identificó un valor predictivo, establece una base sólida para futuras investigaciones sobre biomarcadores mitocondriales en DBP.
