Abstract

[Resumen] Presentación del caso: Se describe el caso de un varón de 56 años de edad que acu-dió a la Clínica Universitaria de Podología (CUP) de la Universidad de A Coruña deri-vado de Atención Primaria para realizarle una revisión y tratamiento de alteraciones un-gueales en los pies. Valoración: Presencia de queratodermia palmoplantar de color rojo anaranjado y una eritrodermia consistente con cierta descamación e hiperqueratosis folicular tanto en ex-tremidades, tronco como en área facial. Debido a la extensión del cuadro cutáneo se derivó al Servicio de Dermatología del Hospital Naval de Ferrol. Plan de actuación: Inicialmente en el Servicio de Dermatología se le pautaron emolien-tes y vaselina salicilada al 5% junto prednisona oral; paralelamente en el Servicio de Podología se realiza corte y fresado de uñas y deslaminación de las hiperqueratosis. Posteriormente se observa la ineficacia del tratamiento y se descubre un adenocarci-noma de sigma, por lo que se inició tratamiento con apremilast. Discusión: La pitiriasis rubra pilaris (PRP) puede manifestarse con diferentes presen-taciones clínicas, que marcarán el tratamiento y pronóstico. Además, con la llegada del COVID-19, se observaron casos de PRP asociados al proceso de vacunación de CO-VID-19, o tras dar positivo en pruebas diagnósticas de COVID-19. Valoración: Presenza de queratodermia palmoplantar de cor vermello laranxado e eri-trodermia consistente con algunha descamación e hiperqueratose folicular nas extremi-dades, tronco e zona facial. Pola extensión da afección cutánea foi derivado ao Servizo de Dermatoloxía do Hospital Naval de Ferrol. Plan de actuación: Inicialmente, no Servizo de Dermatoloxía prescribíronse emolientes e vaselina salicilada ao 5%, asociados a un réxime oral de prednisona; Paralelamente, no Servizo de Podoloxía realizase o corte e fresado de uñas e a delaminación da hiperqueratose presente. Posteriormente, observouse a ineficacia do tratamento e descu-briuse un adenocarcinoma de sigmoide, para o que se iniciou o tratamento con apremi-last. Discusión: A pitiriasis rubra pilaris (PRP) pode manifestarse con diferentes presenta-cións clínicas, que determinarán o tratamento e o prognóstico. Ademais, coa chegada da COVID-19, observáronse casos de PRP asociados ao proceso de vacinación contra a COVID-19, ou tras dar positivo nas probas diagnósticas de COVID-19. [Abstract] Case presentation: We describe the case of a 56-year-old male referred to the Univer-sity Clinic of Podiatry (CUP) of the University of A Coruña derived from Primary Care for a check-up and treatment of nail changes in his feet. Assessment: Presence of red-orange palmoplantar keratoderma and erythroderma with some scaling and follicular hyperkeratosis in the extremities, trunk, and facial area. Due to the broad extension of the skin condition, he was referred to the Ferrol Naval Hospital Dermatology Service. Action plan: Initially, in the Dermatology Department, emollients, 5% salicylated Vase-line and oral prednisone were prescribed; At the same time, the Podiatry Service per-forms cutting and milling of nails and delamination of hyperkeratosis. Subsequently, the ineffectiveness of the treatment was observed and an adenocarcinoma of the sigmoid was discovered, for which treatment with apremilast was started. Discussion: Pityriasis rubra pilaris (PRP) can manifest with different clinical presenta-tions, which will determine the treatment and prognosis. In addition, with the arrival of COVID-19, cases of PRP associated with the COVID-19 vaccination process were ob-served, or after testing positive for COVID-19 diagnostic tests.
