Eficacia del tratamiento fisioterapéutico en la espondilolistesis: una revisión sistemática

Abstract

[Resumen] Objetivo. El objetivo de este trabajo es identificar cuáles son las técnicas fisioterapéuticas más eficaces para llevar a cabo el tratamiento de los síntomas generados a raíz de una espondilolistesis a nivel lumbar. Material y método. Se ha realizado una revisión sistemática buscando información en las bases de datos Pubmed, PEDro, Cochrane y Scopus. Los criterios de inclusión establecidos fueron: artículos sobre el tratamiento de la espondilolistesis lumbar, de los últimos 5 años, publicados en inglés, español o portugués. Incluyéndose: metaanálisis, revisiones sistemáticas (RS), ensayos clínicos, ensayos clínicos aleatorizados (ECA) y guías de práctica clínica. Resultados. Después de su búsqueda y selección, se han incluido un total de 8 artículos (5 ECA, 1 RS, 1 Ensayo clínico y 1 estudio de casos) en esta revisión. 4 artículos que hablan sobre el tratamiento en pacientes con dolor debido a la espondilolistesis, 3 artículos sobre la rehabilitación posquirúrgica debida a una espondilolistesis y 1 sobre la preparación preoperatoria. La mayoría de los estudios, enfocan la rehabilitación en la realización de un programa de ejercicios terapéuticos y de terapia cognitivo-conductual para tratar el dolor, mejorar la funcionalidad y manejar el miedo al movimiento. Conclusiones. Existe evidencia de qué en pacientes con dolor lumbar, debido a una espondilolistesis, el ejercicio terapéutico es eficaz en la disminución del dolor y del grado de discapacidad. Además, la terapia cognitiva-conductual (TCC) es eficaz para mejorar la kinesiofobia, la calidad de vida y la adhesión de los pacientes a los tratamientos.

O obxectivo deste traballo é identificar cales son as técnicas fisioterapéuticas máis eficaces para levar a cabo o tratamento dos síntomas xerados como consecuencia dunha espondilolistesis a nivel lumbar Material e método. Levouse a cabo unha revisión sistemática na procura de información nas bases de datos Pubmed, PEDro, Cochrane e Scopus. Os criterios de inclusión establecidos foron: artigos sobre o tratamento de espondilolistesis lumbar, dos últimos 5 anos, publicados en inglés español ou portugués. Incluíndo: metaanálises, revisión sistemáticas (SR), ensaios clínicos, ensayos clínicos aleatorios (ECA) e guías de práctica clínica. Resultados. Tras a busca e selección, incluíronse 8 artigos (5 RCT, 1 SR, 1 ensaio clínico e 1 caso de estudo) nesta revisión. 4 artigos que falan do tratamento en pacientes con dor por espondilolistesis, 3 artigos sobre rehabilitación postquirúrxica debido a espondilolistesis e 1 sobre preparación preoperatoria. A maioría dos estudos abordan a rehabilitación realizando un programa de exercicios terapéuticos e terapia cognitivo-conductual para tratar a dor, mellorar a función e controlar o medo ao movemento. Conclusións. Hai evidencias de que en pacientes con dor lumbar, debido á espondilolistesis, o exercicio terapéutico é eficaz para reducir a dor e o grao de discapacidade. Ademais, a terapia cognitivo-conductual (TCC) é eficaz para mellorar a kinesiofobia, a calidade de vida e a adhesión dos pacientes aos tratamentos.

[Abstract] Objetive. The objective of this work is to identify which are the most effective physiotherapeutic techniques to carry out the treatment of the symptoms generated as a result of a spondylolisthesis at lumbar level. Methods. A systematic review has been performed searching for information in the Pubmed, PEDro, Cochrane and Scopus databases. The established inclusion criteria were: articles on the treatment of lumbar spondylolisthesis, from the last 5 years, published in English, Spanish or Portuguese. Including: meta-analysis, systematic reviews (SR), clinical trials, randomized clinical trials (RCTs) and clinical practice guidelines. Outcomes. After your search and selection, a total of 8 articles (5 ECA, 1 RS, 1 clinical trial and 1 case study) have been included in this review. 4 articles discussing treatment in patients with spondylolisthesis pain, 3 articles on post-surgical rehabilitation due to spondylolisthesis and 1 on preoperative preparation. Most studies approach rehabilitation by conducting a program of therapeutic exercises and cognitive-behavioral therapy to treat pain, improve function, and manage fear of movement. Conclusions. There is evidence that in patients with low-back pain, due to spondylolisthesis, therapeutic exercise is effective in reducing pain and the degree of disability. In addition, cognitive behavioural therapy is effective in improving kinesiophobia, quality of life and patient adherence to treatments
