Abstract

[Resumen] Introducción: La enfermedad renal crónica se define como: la pérdida progresiva, permanente e irreversible de la tasa de filtración glomerular a lo largo de un tiempo variable, expresada por una reducción del aclaramiento de creatinina estimado < 60 ml/min/1,73 m2; la presencia de daño renal persistente durante al menos 3 meses, secundario a la reducción lenta, progresiva e irreversible del número de nefronas con el consecuente síndrome clínico derivado en la incapacidad renal para llevar a cabo sus funciones. Cuando se padece una insuficiencia renal crónica es necesario comenzar con un tratamiento sustitutivo. Existen varias posibilidades, en este caso, se realizó un trasplante renal de donante vivo. Objetivo: Elaborar y presentar un plan de cuidados personalizado para un paciente con una insuficiencia renal crónica que es sometido a un trasplante renal de donante vivo, y que permita ofrecer unos cuidados eficaces y de calidad. Desarrollo: La valoración de enfermería se realizó de manera presencial aplicando el modelo de V. Henderson y empleando las taxonomías enfermeras NANDA, NOC y NIC. Discusión y conclusiones: El plan de cuidados se elaboró teniendo en cuenta las características personales del paciente y resultó efectivo ya que se consiguieron los objetivos propuestos. Como conclusión, destacar la importancia de la enfermera en el ámbito de la educación sanitaria, ya que de esta manera se podría reducir la prevalencia de la enfermedad.

Cando se padece unha insuficiencia renal crónica é necesario comenzar cun tratamento substitutivo. Existen varias posibilidades, neste caso, realizouse un transplante renal de doante vivo. Obxectivo: Elaborar e presentar un plan de coidados persoalizado para un paciente cunha insuficiencia renal crónica que é sometido a un transplante renal de doante vivo, e que permita ofrecer uns coidados eficaces e de calidade. Desenvolvemento: A valoración de enfermería realizouse de maneira presencial aplicando o modelo V. Henderson e empregando as taxonomías enfermeiras NANDA, NOC e NIC. Discusión e conclusións: O plan de coidados elaborouse tendo en conta as características persoais do paciente e resultou efectivo xa que se conseguiron os obxectivos propostos. Como conclusión, destacar a importancia enfermeira no ámbito da educación sanitaria, xa que desta maneira poderíase reducir a prevalencia da enfermidade.

[Abstract] Introduction: Chronic kidney disease is defined as the progressie, permanent and irreversible loss of the glomerular filtration rate over a variable time, expressed by a reduction in estimated creatinine clearance <.60 ml/min/1,73 m2; the presence of persistent renal damage for al least 3 months, secondary to the slow, progressie and irreersible reduction of the number the nephrons with the consequent clinical syndrome resulting in renal disability to carry out their functions. When suffering from chronic renal failure it is necessary to start with a treatment of replacement. There are several possibilities, in this case, a living donor kidney transplant was performed. Objective: Develop and present a personalized care plan for a patient with chronic renal failure who undergoes a kidney transplant from an alive donor, and that can offer effective and quality care. Development: Nursing assessment was done face-to-face applying the V. Henderson model and using the NANDA, NOC and NIC taxonomies. Discussion and conclusion: The care plan was developed taking into account the personal characteristics of the patient and was effective because the objectives were achieved. In conclusion, highlight the nurse importance in the field of health education, so their role is important in reducing the disease prevalence.
