Abstract

[Resumen] Con este estudio se pretende conocer la presencia de 6 genes que codifican para enzimas modificadoras de aminoglucósidos (AMEs), en 151 cepas clínicas procedentes de 34 hospitales de todo el territorio español. El estudio de la presencia o ausencia de dichos genes se ha llevado a cabo mediante la técnica de PCR, con posterior secuenciación de los fragmentos obtenidos, con el objeto de confirmar la amplificación. Los resultados obtenidos son similares a los esperados con una alta prevalencia del gen aac(3)-Ia asociado a la resistencia a gentamicina, antibiótico con el mayor número de resistencias dentro de las cepas analizadas; siendo la combinación más frecuente aac(3)-Ia con aph(3)-Ia seguida por la combinación de aac(3)-Ia con aac(6)-Ib, en el caso de combinación de dos genes y aac(3)-Ia, aac(6)-Ib y aph(3)-Ia en el caso de las combinaciones de tres genes. Tanto las combinaciones de genes, así como los valores de expresión de los genes observados en estas cepas se asemejan a las expuestas por otros investigadores en sus respectivos trabajos. Algunas de las cepas muestran un resultado anómalo, pero podría explicarse por la presencia de otros mecanismos de resistencia, mutaciones o bien por un fallo durante el proceso de estudio de resistencia. A pesar de los datos anómalos se ha demostrado claramente la amplia distribución de estos genes de resistencia y la importancia de su seguimiento y control no solo dentro del ámbito nacional, sino también en el resto del mundo.

[Abstract] In this study it is studied if there is any gen which codifies for amynoglucoside modifying enzymes in the 151 clinical isolates for 34 Spanish hospitals. The study of the presence or not of these genes has been done by PCR assays and in the cases of correct amplifications our samples were sent to sequencing program in order to confirm the results. The results obtained were similar to the expected ones with prevalence of aac(3)-Ia gen, which is associated with gentamicin resistant phenotype. This antibiotic is the one which has the most resistant assays in our study. The majority gen combination was aac(3)-Ia with aph(3)-Ia, followed by the combination of aac(3)-Ia with aac(6)-Ib; the three genes most frequent combination was aac(3)-Ia, aac(6)-Ib and aph(3)-Ia. The genes combinations and the number of assays with any resistant gen, which were observe in this study, have high correlation with the results that other researchers obtained in their experiments. In spite of some of the isolates have a different result than expected. Nevertheless, such results can be explained with the presence of other resistant mechanisms, mutations or possible errors during the resistant study in these isolates. However, it is undoubtedly shown that there is a wide presence of these resistant genes in Spain and the importance of vigilance and control of these resistant bacteria in this country and worldwide.

[Resumo] Con este estudo pretendíase coñecer a presenza de 6 xenes que codifican para encimas modificadoras de aminoglucósidos (AMEs), en 151 cepas clínicas procedentes de 34 hospitais de todo o territorio español. O estudo da presenza ou ausencia dos devanditos xenes levouse a cabo mediante a técnica de PCR e con posterior secuenciación dos fragmentos obtidos, co obxecto de confirmar a amplificación. Os resultados obtidos son similares aos esperados cunha alta prevalencia do xene aac(3)-Ia asociado á resistencia a gentamicina, antibiótico co maior número de resistencias dentro das cepas analizadas; sendo a combinación máis frecuente aac(3)-Ia con aph(3)-Ia seguida pola combinación de aac(3)-Ia con aac(6)-Ib, no caso de combinación de dous xenes e aac(3)-Ia, aac(6)-Ib e aph(3)-Ia no caso das combinacións de tres xenes. Tanto as combinacións de xenes, así como os valores de expresión dos xenes observados nestas cepas aseméllanse ás expostas por outros investigadores nos seus respectivos traballos. Algunhas das cepas mostran un resultado anómalo, pero podería explicarse pola presenza doutros mecanismos de resistencia, mutacións ou ben por un fallo durante o proceso de estudo de resistencia. A pesar dos datos anómalos demostrouse claramente a ampla distribución destes xenes de resistencia e a importancia do seu seguimento e control non só dentro do ámbito nacional, senón tamén no resto do mundo.
