Cálculo de lentes intraoculares

Abstract

Para el cálculo de la potencia de la lente intraocular (LIO), la precisión de las medidas biométricas es fundamental para evitar resultados refractivos postoperatorios sorpresivos. Existen diferentes fórmulas de cálculo empíricas y teóricas. Con correcciones y modificaciones en casos de ojos no standard. A día de hoy, las fórmulas más conocidas son de 3ª y 4ª generación, como la SRK/T, Hoffer Q, Holladay I y Holladay II. Ésta última siendo más evolucionada, basando sus cálculos en más parámetros que las de tercera generación. Este trabajo tiene como objetivo desglosar el proceso de obtención de una potencia de LIO emetropizante lo más exacta posible. Para ello, se han estudiado los métodos de cálculo empíricos y teóricos, comparando su exactitud en ojos normales y su variación en casos fuera de lo normal. También se han identificado las principales fuentes de error y su resolución. Como resultado, se ha demostrado que la predicción de la ELP (posición efectiva de la lente tras la cirugía de cataratas) es, junto a la longitud axial, uno de los parámetros más influyentes en el cálculo de la LIO. Se han estudiado las alternativas de cálculo para casos de cirugía refractiva previa, y se ha confirmado que las fórmulas teóricas de 3ª y 4ª generación son las más utilizadas hoy en día en el cálculo de la LIO
