Abstract

La decisión de bioequivalencia depende directamente del criterio utilizado para establecer tal condición. Las principales agencias del medicamento la Food and Drug Administration (FDA) y la European Medicines Agency (EMEA) aceptan la bioequivalencia cuando el intervalo de confianza del 90% para la razón, prueba/referencia se encuentra dentro del intervalo del 80%- 125%. Sin embargo, cuando existe una variabilidad elevada tales condiciones dificultan la determinación de la bioequivalencia. Por ello, tanto la FDA como la EMEA han presentado propuestas las cuales permiten ampliar los límites de equivalencia en determinadas condiciones. Aunque comparten algunas características difieren en la metodología de los límites escalados que determinan la bioequivalencia. Para realizar la comparación entre los criterios de la FDA y EMEA, se ha llevado a cabo un estudio de simulación para diferentes diseños cruzados, distintas varianzas residuales, tamaños de muestra y efectos de tratamiento

Recentment, l'agència europea del medicament, EMEA, i la FDA nord-americana han publicat directrius sobre bioequivalència en drogues d'alta variabilitat. La hipòtesi de partida del treball és que no estan ben fonamentades estadísticament, particularment l'europea. Es proposa un estudi de les seves propietats, especialment de l¿error de tipus I (que en aquest cas correspon al risc d'usuari, donar per vàlid un genèric que en realitat no és bioequivalent al medicament de marca) i fer propostes per a corregir les deficiències observades, si n'hi ha.
