Perioperativ överrapportering-En systematisk litteraturstudie

Abstract

Bakgrund: En patient som ska opereras möter många vårdare i olika instanser i vårdkedjan inför, under och efter sin operation. En vanlig avvikelse inom vården är bristande kommunikation och den muntliga rapporteringen är ett sådant tillfälle. Syftet är att beskriva aktuellt kunskapsläge gällande perioperativ överrapportering för patientens rätt till säker vård. Studien är en systematisk litteraturstudie (pilotstudie). Resultat: Överrapporteringen var ofta informell, ostrukturerad och inkonsekvent. Med ett standardiserat sätt att arbeta förbättrades patientsäkerheten och teamarbetet stärktes. Slutsats: Slutsatsen är att strukturerad överrapportering ger säkrare vård för patienten och bättre förståelse i teamen genom ökat deltagande. Operationssjuksköterskan kan som en av de ansvariga i vårdkedjan vid överrapportering stärka patientens delaktighet och ansvara för en säker och korrekt vård genom att använda ett standardiserat sätt för överrapportering.

Background: A patient undergoing surgery will meet many caregivers in different settings pre-, intra- and postoperative in the surgical pathway. Lack of communication is a common digression and the handoff is such an occasion. The purpose is to describe current knowledge about the patients´ right to safe care perioperative. The study is a systematic review (pilot). Result: The handoff was informal, unstructured and inconsistent. To work in a standardized way improved the patient saftey and strengthened the teamwork. Conclusion: The conclusion is that a standardized handoff provides safer care for the patient and improves the understanding in the team through higher participation. The theater nurse can, as a part in the pathway of care, strengthen the patients participation and be responsible for a safe and correct care through a standardized handoff.
