Depressionens lidande. Patientens upplevelse av lidande och lindrat lidande

Abstract

Depression är ett av Sveriges största folkhälsoproblem och beskrivs som ett stort psykiskt lidande. Att lindra lidande är ett av sjuksköterskans grundläggande ansvarsområden. Syftet med detta fördjupningsarbete i omvårdnad var att få fördjupad förståelse för hur patienter med depression upplever lidande och hur sjuksköterskan kan lindra lidandet. Katie Eriksson beskriver tre kategorier av lidande i vården: sjukdomslidande, vårdlidande och livslidande. Resultatet är byggt på vetenskapliga artiklar som söktes manuellt och i databaserna CINAHL och PsycINFO och tolkades utifrån Erikssons lidandekategorier. I sjukdomslidandet var fysisk smärta och skuld- och skamkänslor dominerande. Ouppfyllda förväntningar på sjuksköterskans närvaro karaktäriserade vårdlidandet. Livslidandet präglade patienternas hela liv och flera identifierade sig med depressionen och såg ingen möjlighet att separera sig ifrån den. I diskussionen tolkades lindrat lidande utifrån Erikssons teori om lidandets drama vilket medförde en fördjupad förståelse för hur sjuksköterskan kan lindra lidandet. Tolkningen visade att patienterna önskade att sjuksköterskan skulle agera aktivt genom främst bekräftelse och närvaro. Försoningen med lidandet visade sig vara en i huvudsak personlig process där sjuksköterskans närvaro inte var lika påtaglig. Vår slutsats är att teorin om lidande är användbar i omvårdnad av patienter med depression och kan underlätta i sjuksköterskans förståelse för patientens lidande.
