Exención de estudios de bioequivalencia para nuevos medicamentos genéricos

Abstract

Los medicamentos genéricos deben demostrar su bioequivalencia con el comparador para poder asegurar su intercambio, y requieren para ello la realización de estudios en voluntarios sanos. El Sistema de Clasificación Biofarmacéutica permite clasificar a los fármacos considerando los parámetros de solubilidad acuosa y permeabilidad intestinal, y plantea la posibilidad de establecer correlaciones in vitro-in vivo que permitan sustituir los ensayos en humanos por ensayos de disolución in vitro, para fármacos que se presenten como formas farmacéuticas orales sólidas de liberación inmediata. Se puede solicitar bioexención para los fármacos de Clase I; para que se de la posibilidad respecto a los fármacos de Clase II debe tratarse de ácidos débiles, el medicamento debe ser de disolución rápida y los perfiles de disolución entre el comparador y el comparado muy similares en el rango de pH establecido. La exención de estudios de bioequivalencia in vivo en el caso de productos que contengan fármacos de Clase III debe realizarse individualmente, prestando especial cuidado. Con los fármacos de Clase IV difícilmente se pueden obtener correlaciones in vitro-in vivo.
