Nuevas dianas terapéuticas en el tratamiento de la isquemia cerebral

Abstract

La isquemia cerebral aguda se define como una enfermedad neurológica que ocurre por una disminución o bloqueo del flujo sanguíneo a parte del tejido cerebral. El síndrome más frecuente es el producido por la oclusión de la arteria cerebral media ACM (Medial cerebral arterial oclusión MACAO).La isquemia cerebral va a producirse por una reducción sostenida del flujo sanguíneo cerebral por debajo de 10 ml/mg/min que provoca necrosis cerebral con liberación de glutamato y entrada de calcio en las neuronas mediada por receptores NMDA de glutamato. Este bloqueo del flujo sanguíneo ocasiona la ruptura de los vasos sanguíneos, disminuyendo la cantidad de glucosa y oxígeno. A día de hoy, el tratamiento de elección es el activador tisular de plasminógeno recombinante (rt-PA), el único agente trombolítico disponible que presenta un período de efectividad muy estrecho lo que unido a sus exigentes riesgos de inclusión y sus efectos secundarios neurotóxicos hacen complicado su uso clínico. Un período de isquemia cerebral transitoria de 10 min puede producir un profundo daño neuronal que solo es evidente 3 días post-ictus y continúa progresivamente en los meses posteriores. Un alivio de la oclusión vascular es el primer método para tratar la isquemia, pero la instauración del daño independientemente de la recuperación del flujo sanguíneo hace necesaria la búsqueda de otras alternativas.
