Abstract

Malign melanom mortalitesi ve morbiditesi en kötü deri tümörü olup melanin üreten hücrelerin malign transformasyonu sonucu gelişmiştir. Deri kanserleri arasında 3. sıklıkla görülmekte ve melanom insidansı her yıl artış göstermektedir. Malign melanomlarda bugün için en etkili tedavi geniş cerrahi eksizyon ve gerekirse bölgesel lenf bezi eksizyon/diseksiyonudur. Malign melanomun klinik, histolojik ve biyolojik özelliklerinin daha iyi bilinmesi tümörün erken tanı ve tedavisindeki gelişimlere katkı sağlayacaktır. Sonuçta, tüm kanser türlerinde olduğu gibi melanomda da erken teşhis ve tedavi yaşam süresini artırmaktadır. . Çalışmamız tedavi alternatifleri oldukça kısıtlı olan metastatik malign melanom hastalarında hastalığın seyri ve prognozu hakkında bilgi sağlayarak tedavi protokolü seçiminde ve hastalığın yönetimi hakkında fayda sağlamayı hedeflemektedir. Biz bu çalışmada malign melanom olgularının patoloji materyallerinde yapılacak analizle c-myc onkogeni ve matriks metolloproteninaz-2 ekspresyonunun malign melanom hastalarında metastaza ve prognoza olan etkisini değerlendirmeyi amaçladık . Çalışmamıza 2006-2015 yılları arasındaki Mersin Üniversitesi Plastik Rekonstrüktif ve Estetik Cerrahi kliniğinde tedavi gören 63 hasta dahil edildi. Tüm malign melanom olguları retrospektif taranarak yaş, cinsiyet, lezyonun yerleşimi, histolojİk tipi, evrelemesi ve lenf nodu tutulumu açısından değerlendirildi. Uygun olan 47 hastanın spesmenleri yapılan immunohistokimyasal boyamayla c-myc ve MMP-2 enzim ekspresyonu açısından incelendi. Sonuçların metastaz, sağkalım sürelerine olan etkisi istatistiksel olarak analiz edildi. Her ikisininde ayrı ayrı pozitifliği takip süresini azalttığı, MMP-2 pozitifliğinin sağkalımı olumsuz etkilediği ve ölüm riskini 4 kat arttırdığını gördük. (p:0,022) Çalışmamız c-myc ve MMP-2 kötü prognoz kriteri olarak değerlendirilebileceğini kanıtlar niteliktedir. Anahtar kelimeler: c-myc, malign melanom, MMP-2

Analyzing the effect of c-myc oncogene and matrix mettalloproteinase-2 enzyme ekspression on metastasis and prognosis of malignant melanoma Malignant melanoma which has the highest mortality and morbidity is the worst skin tümör. İt is derived from malignant transformation of melanin producing cells. The incidence of the melanoma has increased over the past several decades. Most efficent terapy for the melanom is surgical eksizion and lymph node disection, to understand the histologıcal, clinical and bıologıcal propertıes of malignant melanoma is importand because ıt will be helpfull for the developement of early diagnosis and new terapeutıc pathways. Early diagnosis and treatment increase the survival rate of melanoma and also all cancer types as well. The aim of this study is to measure the the effect of c-myc oncogene and matrix mettalloproteinase-2 enzyme ekspression on metastasis and prognosis of malignant melanoma. At the end of this study we hope to have informatıon about the prognosis of melanoma so to be helpfull for choosing the treatment strategy. 63 patients treated in Mersin University Plastic Reconstructive and Aesthetic Surgery clinic durıng 2006-2015 were included to this study. All clinical and histological parameters were collected from each patient's records and survival rate is assessed. 47 suıtable tümor specimens were assesed by immunohystochemical stains and analyzed for the c-myc and MMP-2 positivity. All results were evaluated statistically for the effect on melonoma metastasis and survival rate. C-myc positivity and MMP-2 positivity decreases the survival rate of melanoma statistically significant.(p:0,022) MMP-2 positifity increases the risk of death 4 times more. So this study confirms that c-myc and MMP-2 can accept as a prognostic factor. Keywords : c-myc, malignant melanoma, MMP-2
